A CA 19-9 blood test measures the level of a tumor marker protein in your blood. It is mainly used to monitor certain cancers of the digestive tract (especially pancreatic cancer), assess treatment effectiveness, help predict prognosis, and detect recurrence. It is not used alone for screening or diagnosis.

Why this test may be ordered
- To monitor cancer treatment and see if it is working.
- To help make a prognosis by determining cancer stage and likelihood of treatment response.
- To check whether cancer has returned after treatment.
- If you have symptoms such as jaundice, weight loss, abdominal pain, or a swollen gallbladder or liver that may be linked to cancer.
Preparation and collection
No special preparation is usually needed. However, ask your provider if you need to stop taking certain vitamins before the test.
A health care professional will take a blood sample from a vein in your arm using a small needle. The blood is collected into a test tube or vial. You may feel a sting when the needle goes in or out. The process usually takes less than five minutes.
There is very little risk to having a blood test. You may have slight pain or bruising at the spot where the needle was inserted, but most symptoms go away quickly.
Understanding results
If you are being treated for cancer, your provider will track changes in your CA 19-9 levels over time to assess treatment response. The results may also be used to help predict prognosis or detect recurrence.
- CA 19-9 levels are increasing: Your tumor may be growing or your treatment may not be working effectively. Limitation: Other factors (such as inflammation or benign conditions) can also cause levels to rise, so more tests are needed to confirm. Next step: Your provider will likely order additional imaging or other tests to evaluate.
- CA 19-9 levels are decreasing: Your tumor may be shrinking and treatment may be working. Limitation: A decrease does not guarantee the tumor is gone; further monitoring is needed. Next step: Continue your treatment as recommended by your provider.
- CA 19-9 levels have stayed the same: Your disease may be stable, with no significant change. Limitation: Stable levels do not rule out slow progression; ongoing monitoring is important. Next step: Your provider will continue to monitor your condition with scheduled tests.
- CA 19-9 levels decreased after treatment but later increased: Your cancer may have returned or continued to grow. Limitation: Higher levels can also be caused by noncancerous conditions, so additional tests are required to confirm. Next step: Your provider will order more tests to determine if cancer is present or if another cause exists.
- High CA 19-9 levels can be caused by noncancerous conditions such as gallstones, pancreatitis, cirrhosis, or liver disease.
- Some people do not produce CA 19-9 even when they have a cancer that usually causes high levels, making the test unhelpful for them.
- Different labs may use different test methods, which can affect results. Consistency in lab testing is important for accurate monitoring.
- A CA 19-9 test cannot be used alone to screen for or diagnose cancer; it must be combined with other tests and clinical evaluation.
